The box is plastic, very shallow and attaches in
the middle to a joist. The weight of the fan is carried by 2 #12 1-1/2"
screws which go directly into the joist. Although the screw spacing is
the same as the much smaller screws for a light fixture junction box
there is no crossbar and the screws go directly into the joist.
Quick and dirty. These are called pancake boxes. They are legal but are
generally used for other applications. Not to hang any weight. Basically
it saved the contractor 15-20 minutes and passed inspection. He could
have at least used a metal one.
Note that your "modification" is not legal as it defeats the purpose of
the box. But not much choice now. If corners hadn't been cut you
wouldn't have had to go there.
