That is exactly the device that I bought. It comes with an adapter plate which
fits in the top clamp. I can safely get 20mm of offset, but really need up to
30mm to be certain of getting nodal point of any lenses I am likely to use. I
think it will do the m 4/3 12 - 50mm as well as my Olympus tilt/shift lens. My
60mm macro would be pushing it, but my M Leica 90mm f2.8 should be no problem.
I shall just keep an eye open for a grooved slide in Arca Swiss fittment.

I do have a couple of ball heads but the ball heads introduce another
problem. The ball head gives you a simple method of having a level
mounting surface but cannot itself be used as a rotation point for the
pano. That would require adding a rotating mounting surface on top of
the ball head such as this

which has already been mentioned here a couple of times.
I don't remember who but I think someone spoke of using two of these to
get shifts in 2 directions but I didn't understand how they were to be
put together. Perhaps the second head has to be attached to some form
of Arca Swiss sliding plate riding in the grooves of the first head.
