I suspect that many didn't take very good care of the cases, as they are of
little value to a user. I have noted that they are not as well made as the
contents, tending to come apart at the glue joints, and others succumb to
mold and fungus. The only reason I saved any was to have them for resale.

Probably less money-grubbing than you suspect.
All new Olympus lenses I've ever purchased came with a case or a pouch. I
generally have little need for them, and set them aside, and either include
them when I get rid of the lens, or (more likely) offer them free for cost
of shipping on this list.
