Someone looked at velvia under a very high power microscope and posted a
web page explaining that pepper grain was caused by small evenly
dispersed bubbles in a scratch resistant layer of the film. If I can
find the link I'll post it.

When velvia is scanned you can get numerous tiny black dots in the
image.
