If the flash won't fire at 1/60 on shoe 1, it's most likely an X sync
circuit problem as pointed out by JH. This could be down to a number of
reasons, but basically if the shutter speed is accurate, the X circuit
microswitches are not timed correctly. If the shutter speed is not accurate,
have it adjusted and then see above.
Because 1/60 is close to the limit for the shutter assembly's natural X sync
speed, the actual speed would not have to be out by much to prevent both
microswitches closing at the nominal speed.
The quench - signal problem is more likely to cause the "fires at a fraction
of max output" situation.
I've never seen documentation of this issue. I pieced the scenario together
from various sources, including the OM2 circuit diagrams and Mannheim's
book. The result being that I don't know any more about it than I have
already written! Sorry!
