You must be talking about the camera's built-in flash, then.
You might consider trying a ring flash. They are the standard for
medical/dental photography. Even, shadowless illumination.
Olympus made superb ring flashes for TTL metering with the OM bodies. I don't
know what TTL units are available for digitals, but there are several
third-party ring flashes out there.
