Really tiny dust happens, but I've seen major flecks in lenses.
Strange thing, though. I've never seen the flecks in a
silver-nosed lens, only the newer black-nosed wonder-weenie
multicoated jobbies. I'd guess that Olympus changed the black
paint somewhere through the years.
