The collars were never available separately to my knowledge. If we don't
have an original collar, we've found a part that will suffice, though it is
chrome instead of black. But hey, it works....
Mark's technique (round hex used as a punch) sounds excellent, BTW. It's
tough to get an attractive bond with cyano-glue, though I have been known to
use a dab of it at the corners now and then to reinforce a replaced collar.
Often I will put a thin ring of JBWeld around the underside just in case, as
well.
