Yes SLR can be off, I had a Nikon FA, two OM with focusing system off
problem (in over SLR 15 bodies checked). All my gears (including range
finder) were check and adjusted with the following method:

(I have to update the picture, OM FS can place 90 deg on the guide
rail, no need to make an angle)
It works very well for me, I have every picture in exact focus (if I
was careful and not in hurry). But make sure you use ground glass for
focusing, the trick is turning the focusing ring left-right a few time
with reduce traveling then you will feel where the centre (focus
point) is.
