Given the different behavior you see based upon the positioning of the
mode switch perhaps it's nothing more complicated than dirty or bent
brush contacts in the mode switch.
However, before opening up the top of your OM-4 see the 1999 archives
for "Hot Shoe Problem". You can probably get the top off easily but
getting it back on with all the wires stuffed into the proper place
without breaking them or doing further damage elsewhere is a point for
careful consideration.
