Every working Zuiko can focus to infinity. Do you crosschecked your
camera (screen, etc.)

No, it is not. This should not to be seen, exept you have an very
demanding lenstest.

Send it back, maybe somebody has disasembeled it and incorrectly
reassembled it.

I use it with the 200mm Zuiko and it is realy good. Olympus claims in
the Olympus Magazin (OM 2/81) that it is of such good quality, that
there is no need of stoping down if used with the dedicated lenses (2.8
100mm, 2.8 3.5 135mm, 4.0 5.0 200mm and probably the 5.0 100-200mm Zoom,
which was not avaible in 1981). They write further, that it should not
be used with the 180mm, 75-150mm, 85-250mm and lenses below 85mm. The
leaflet, which comes with the 2.0 100mm, says, that the 2x converter
should not to be used with the lens.
