Generally I agree with Ken (not that it's important to this discussion, I
have nearly 6 million miles in FFB programs, so have a bit of perspective.)
I do think the airlines have belatedly adopted a reasonable policy
approach: once on the airplane, not to be taken off. That is, solve
any problems before boarding.
In the unhappy event someone does have to get off, I recall Air Canada's
approach late one night in Montreal: they announced that one passenger
needed to deplane , and that no one had yet volunteered. So, they said,
we'll wait. If someone doesn't volunteer, we simply won't go and will
cancel the flight. Perhaps the appeal to group welfare works better in
Canada ... but it sure did work ... and as far as I could tell, no hard
feelings.
