That is confirmed by my recollection of a conversation with a specialist OM
repairer over here maybe ten years ago.  He had just replaced the rear
element of an 8mm, having secured a replacement element from Olympus UK. He
was a little surprised to hear (from me) that John H. had reported they were
no longer available in US or Japan, and cursed that if he had only known, he
would have sent the replaced elements out to Optical Instruments Balham for
polishing and re-coating. I'm not sure if he even searched the bins just in
case.

He couldn't do that now, unfortunately, OIB closed two years ago.

No. The rear doublet is very small and odd but doesn't suffer this way. The
problem occurs in the space between that and the next element - the two
opposing surfaces haze over and become etched - they can be repolished and
single recoated by specialist services. As to what causes it in just one
specific location - that's a mystery. There's no lubricant in there (the
lens focuses by a simple helical that moves the whole lens unit forward) and
the elements are held in place by simple spacer rings in a tube. My bet is a
failure of specialised coatings on just those two surfaces.
