I had an OM4 + 35-80/2.8 that had a similar problem. Yes the lens can fall
off. I caught it on the way down, so no harm was done. Both camera and lens
visited one of the two great OM techs on the list. Although the lens has a
slight bit of play on that body, it won't accidentally fall off. In my case
other lenses worked fine on that body. The problem lens was fine on other
bodies. It was just that specific combination that didn't work.
The notch the catch fits in was slightly rounded.

Just purchased a clean second-hand 135/3.5 lens and was wondering if any of
you experienced the same problem.....Mounted the lens on the camera and
discovered the lens release button is not working and the lens doesn't lock
into place. Under normal usage is the any real chance of the lens damaging
the camera body or loosening itself enough to fall out?
