That is a pre OM-System 200/4 for the M42 screwmount Olympus FTL SLR.
The general build is much like the Pen F lenses of the day. In fact the lens
is a near swollen twin for the 150/4 Pen F/FT lens. I think that the bodies
were made by Miranda, but I don't know about the lenses.
