A thumb-screw can also have a flattened paddle end, grippable and turnable
between thumb and fingers. This is where there is clearance around the screw
for the larger paddle end. A way of getting around this clearance
requirement is to make the grippable end a folding wire loop, which is
folded down when not being turned. I have this on the shift levers of a
bicycle, for adjusting lever tension.
