I had this problem on my 28 2.8.
I undid the three screws on the mount and took it off, removed the depth
preview mechanism and cleaned it in meths. Then reassembled it, and now its
fine. I think it was just dirt.
It did work fine using the camera operated lever on the back, so I guessed that
the diaphragm itself wasn't sticky. Does it work OK when it's on the camera?
