Dan,
You may be suffering from an extremely shallow depth of field focusing with
the lens wide open, particularly if it's a long, fast lens very close
focused. The DOF with an 85mm - 135mm at f/2 or f/2.8 with a subject about
one to two feet from the lens can be a fraction of an inch. Unless the
subject is dead flat, it can be impossible to make all of it in focus. Try
stopping down and using the DOF preview button. Sometimes I'll turn the
split image across an edge at the distance I want to focus, focus the lens
with it, and then compose the frame.
