The offending mounts are labeled OM, the phillips screw, one of
four, crosses the little plastic reset teat on the mount flange and in
time will wear it down until it no longer functions. The "good"
Tamron adaptall 2 mounts are labeled OL. Check first by mounting
on the OM4T to see if the screw passes over the teat. If it does,
just epoxy and sand smooth the phillips head screw on the mount.
