The most commom way is to cradle the lens in my left palm, support
wooden stick with 2 fingers of left hand, swing hammer with right hand. 
That's a sophisticated as it gets.
Sorry to burst bubbles but fancy supports simply aren't bothered with
(from my Nikon and Olympus factory experience.)
